transport: use an intrusive linked list for per-stream data frame queues (#9490)

#9444 replaced the linked lists in the control buffer and in each
stream's queue of data frames with ring buffers. This works well for the
control buffer, which is long lived and shared by all streams. For
per-stream queues, however, the ring buffer allocates a backing array on
each stream's first enqueue, which costs one allocation per stream on
both the client and the server. This buffer can also pin [up to 1KB
memory](https://github.com/grpc/grpc-go/pull/9444#discussion_r4157701231)
per stream.

This PR replaces the per-stream ring buffer with an intrusive singly
linked list: `dataFrame` gets a `next` field and `outStream` keeps
`dataHead/dataTail` pointers. A `dataFrame` is only ever queued on one
stream, and the queue is only accessed by the loopy writer, so no extra
synchronization is needed. Adding the field does not change the
allocation size class of `dataFrame` (72 → 80 bytes).

Server trailers, which were previously queued behind the data frames as
a `*serverHeaders` item, are now stored in a separate
`outStream.trailer` field and written once the data queue is empty. As a
result, the queue only holds `*dataFrames` and the type assertions in
`processData` and `updateStreamAfterWrite` are removed.

The control buffer continues to use the ring buffer from #9444.

## Benchmarks

benchmain with -bufconn -networkMode=Local -compression=off
-benchtime=60s.

### Unary, 100 concurrent calls, 0-byte payloads

| Round | QPS | Avg latency | p99 latency | Bytes/op | Allocs/op |
|---|---|---|---|---|---|
| 1 | +2.14% | -2.07% | -2.50% | -1.66% | 137.19 → 135.20 |
| 2 | +1.57% | -1.56% | -1.58% | -1.69% | 137.19 → 135.19 |
| 3 | -0.15% | +0.14% | -0.49% | -1.64% | 137.19 → 135.19 |
| **Mean** | **+1.19%** | **-1.16%** | **-1.52%** | **-1.66% (~158 B)**
| **-2.00** |

The 2 fewer allocations per RPC are the per-stream ring buffer arrays
(one on the client, one on the server).

### Streaming, 200 concurrent calls, 100-byte payloads

| Round | QPS | Avg latency | p99 latency | Bytes/op | Allocs/op |
|---|---|---|---|---|---|
| 1 | +1.32% | -1.17% | -0.99% | 0.00% | 0.00% |
| 2 | +1.72% | -1.61% | -1.04% | 0.00% | 0.00% |
| 3 | +1.24% | -1.33% | -0.69% | 0.00% | 0.00% |
| **Mean** | **+1.43%** | **-1.37%** | **-0.91%** | **0.00%** |
**0.00%** |

Streams are long lived in this workload, so the ring buffer's per-stream
allocation was already negligible.

RELEASE NOTES: N/A
xds/clustermanager: stop splitting endpoints by hierarchical path (#9451)

The `xds_cluster_manager_experimental` LB policy currently calls
`hierarchy.Group()` on the endpoints it receives and gives each child
only the endpoints whose first hierarchical path element matches the
child's name. Endpoints with no path, or with a path that doesn't match
any child, are silently dropped.

This splitting has never done anything in practice:
* The cluster manager's only producer is the xDS resolver, which sends a
resolver state [without any
endpoints](https://github.com/grpc/grpc-go/blob/master/internal/xds/resolver/xds_resolver.go#L390-L395).
So `hierarchy.Group()` always returns an empty map, and every child gets
`nil` endpoints.
* The children are CDS balancers, which get their endpoints from the xDS
config passed in the resolver state attributes, not from
`ResolverState.Endpoints`.

### History
The splitting logic was added in #3746, which introduced the
`xds_routing` balancer. It was copied, along with its comments, from the
`weighted_target` balancer (#3541), where splitting by hierarchical path
is required. `xds_routing` never received endpoints with hierarchical
paths. In #4083 the routing balancer was split into the xDS config
selector and the cluster manager, and the `hierarchy.Group()` call
carried over unchanged.

### Other languages

Neither gRPC C++ nor gRPC Java splits endpoints by hierarchical path in
the cluster manager. Both forward the parent's full address list to
every child and change only the child's LB config:

* C++:
[`XdsClusterManagerLb::UpdateLocked`](https://github.com/grpc/grpc/blob/87963e17a3f5651a28eeebcbfe7d5be78c94d457/src/core/load_balancing/xds/xds_cluster_manager.cc#L285-L294)
passes `args.addresses` to each child, and
[`ClusterChild::UpdateLocked`](https://github.com/grpc/grpc/blob/87963e17a3f5651a28eeebcbfe7d5be78c94d457/src/core/load_balancing/xds/xds_cluster_manager.cc#L453-L473)
forwards them unchanged.
* Java:
[`ClusterManagerLoadBalancer.createChildAddressesMap`](https://github.com/grpc/grpc-java/blob/96e4d30dc00e8ec62b83d27439fc39925f1b6ce0/xds/src/main/java/io/grpc/xds/ClusterManagerLoadBalancer.java#L108-L114)
builds each child's `ResolvedAddresses` from the parent's, overriding
only the LB config. By contrast, Java's `weighted_target` [does call
`AddressFilter.filter`](https://github.com/grpc/grpc-java/blob/96e4d30dc00e8ec62b83d27439fc39925f1b6ce0/xds/src/main/java/io/grpc/xds/WeightedTargetLoadBalancer.java#L94).

### This PR

* Removes the `hierarchy.Group()` call from the cluster manager and
forwards `ResolverState` unchanged to every child.
* Updates the tests that relied on the splitting. The tests now use a
test child policy that delegates to `pick_first` with a backend taken
from its own LB config and ignores the endpoints from its parent. This
mirrors how the real CDS children get their endpoints. The other tests
now send plain endpoints without hierarchical paths.
* Adds `TestClusterManagerForwardsResolverState`, which checks that
every child receives the parent's endpoints and attributes unchanged.

RELEASE NOTES: none
transport: avoid allocating a closure for writeQuota.replenish on every stream (#9493)

`writeQuota.init` stored the bound method value `w.realReplenish` in the
`replenish` func field. Taking a method value that captures its receiver
allocates a closure, so this cost one heap allocation per stream on both
the client and the server, i.e. 2 allocations per unary RPC. The field
only existed so that a test could observe replenishment; production code
calls it in a single place (`loopyWriter.processData`).

This PR makes `replenish` a regular method. The test now detects the
flow-control stall from the receiving side. Before reading, each peer
polls its own stream's inbound flow control (`inFlow.pendingData`,
protected by `fc.mu`) every millisecond until it holds a full window
(`defaultWindowSize`) of received but unread data. The remote sender
can't send more until this side reads and sends a WINDOW_UPDATE, so a
full window means the sender is blocked on stream-level flow control. It
also means the data has actually arrived, not just that loopy has
scheduled it. This replaces the two goroutines that spun on
`runtime.Gosched()` and signalled each other through the
notify/getNotified channels. As a result, the test no longer needs a
hook in production code or any signalling between the client and server,
and it stops burning CPU while it waits.

## Benchmarks

`benchmain` with `-bufconn -networkMode=Local -compression=off
-benchtime=60s`.
extproc: reject invalid server flow control window increments (#9388)

recvFromProcServerLoop applies a ServerWindowUpdate from the external
processor by adding its window_increment value straight into the
per-direction int64 flow control counters, which start at the 64KiB
default. The increment is an int64 the processing server controls, so a
value near math.MaxInt64 wraps the window negative and leaves it there
for good; acquireDownstreamToSidestreamWindow and its upstream sibling
then keep blocking and the RPC's body forwarding through the processor
stalls.

Route both increments through applyServerWindowUpdate, which rejects an
increment that would wrap the int64 window in either direction, failing
the proc stream instead of corrupting the accounting. Negative
increments are allowed, per the spec, and simply shrink the window;
senders block until later updates bring it back above zero. The check
sits next to the window because this recv loop is the only place a
server-supplied increment is applied.

RELEASE NOTES:
* xds: reject flow control window updates from an ext_proc server that
would overflow the window
internal/serviceconfig: reject misplaced signs when parsing durations (#9454)

`Duration.UnmarshalJSON` parsed both the whole and fractional parts of a
duration string with `strconv.ParseInt`, which accepts its own leading
sign. Because the leading `-` had already been stripped, signs in other
positions were accepted and produced wrong values instead of an error:
| Input      | Before   | After |
|------------|----------|-------|
| `"1.+5s"`  | 1.05s    | error |
| `"1.-5s"`  | 950ms    | error |
| `"-1.-5s"` | -950ms   | error |
| `".-5s"`   | -50ms    | error |
| `"--1s"`   | 1s       | error |
| `"-+1s"`   | -1s      | error |

This change strips a single optional leading sign (`-` or `+`) and then
parses both parts with `strconv.ParseUint`, so any further sign is
rejected.
balancer/ringhash: fix flaky TestRingHash_RequestHashKeyConnecting (#9329)

Fixes #9030

`TestRingHash_RequestHashKeyConnecting` counts connection attempts in
the same loop that resumes the blocked one, so it can count an attempt
that its own `Resume()` caused. Resuming makes the endpoint READY,
ring_hash publishes a
picker with `hasEndpointInConnectingState` false, and the queued RPCs
pick again and start a connection to one idle endpoint along the ring
walk, which is what gRFC A76 asks for (picker.go:93-115). If the loop is
still scanning when that
dial reaches the blocking dialer it counts that attempt and the test
fails. Both failures linked from the issue show the second subchannel
being created after the resume, never before it.

The fix orders the phase so the assertion cannot observe something the
test itself caused:

1. First RPC in a goroutine, wait for a hold to be entered.
2. Count all 20 holds, want exactly 1.
3. Second RPC on the test goroutine, short deadline,
`WaitForReady(true)`, want `DeadlineExceeded`.
4. Count all 20 holds again, want exactly 1.
5. Only then resume the single started hold.

Both counts precede any `Resume()`. Only two pickers can exist before
that point and a pick against either cannot start a second connection
attempt, so `at most 1` is tightened to `exactly 1` and the fixed 10ms
sleep is removed. I proved this
with a throwaway instrument that forces the CI interleaving
deterministically (current loop fails 100 of 100, this change 0 of 100),
and also reproduced the flake organically under core pinning (current 2
of 300, this change 0 of 2000).
The CI log ordering, the picker state argument and the full numbers are
in the first comment.

RELEASE NOTES: n/a
xds: fix uint32 overflow scaling rds route runtime_fraction (#9308)

routesProtoToSlice scales a route's runtime_fraction to
parts-per-million by multiplying the numerator by 10000 (HUNDRED) or 100
(TEN_THOUSAND) in uint32. The numerator comes from the control plane, so
a value like 429497 with a HUNDRED denominator computes 429497*10000 and
wraps around to 2704. A route meant to match every request then matches
roughly 0.27% of them, and the rest silently fall through to a different
route action.

Per review feedback, fractional percent handling is now centralized in a
new xdsresource.FractionalPercent type. NewFractionalPercent converts
the proto, computes the parts-per-million value in uint64, caps it at
1000000 (100%), and rejects unsupported denominators. The five places
that parsed the proto themselves now use it:
- the rds runtime_fraction path stores the precomputed PPM (fixing the
overflow above)
- the eds drop policy parser stores a FractionalPercent in
OverloadDropConfig
- clusterimpl reads the PPM directly, replacing its local
dropRequestsPerMillion
- the fault filter's splitPct delegates to it (with a TODO to move
validation to config parsing time)
- ext_authz's parseFilterEnabled delegates to it, replacing the
package-local fraction type

RELEASE NOTES:
* xds: fix a uint32 overflow when scaling a route runtime_fraction that
could make a route match far fewer RPCs than configured, and centralize
FractionalPercent parsing. A route runtime_fraction or an ext_authz
filter_enabled with an unrecognized denominator now causes the resource
to be NACKed.

---------

Co-authored-by: Easwar Swaminathan <easwars@google.com>
grpc: apply the config selector only after the LB policy is updated (#9442)

Per [gRFC
A31](https://github.com/grpc/proposal/blob/master/A31-xds-timeout-support-and-config-selector.md#implementation-for-xds-routeaction-support:~:text=When%20new%20clusters,INTERNAL%20status%20code.),
the channel must update its LB policy before applying a new config
selector, and the LB policy must synchronously provide a new picker
before its update method returns. This ordering guarantees that a config
selector never routes an RPC to a cluster unknown to the current picker.

This PR updates the channel to apply the config selector only after
`updateClientConnState` returns. Because `updateClientConnState` blocks
until the LB policy finishes processing the state update—and
`xds_cluster_manager` produces its new picker before returning—the new
picker is guaranteed to be in place first.

The config selector is then applied while holding `cc.mu`, and the
update is skipped if the channel closed or entered idle mode while the
lock was released. This preserves two invariants:
- The selector reset in `Close()` remains the final write.
- A stale update cannot overwrite the selector installed by a new
resolver after the channel exits idle mode.

Consistent with existing LB policy updates, this relies on the
(undocumented) guarantee that resolvers do not invoke `UpdateState`
concurrently, as overlapping calls could still apply config selectors
out of order.

RELEASE NOTES:
* xds: Fix a race where RPCs could fail with `UNAVAILABLE: unknown
cluster selected for RPC` right after an xDS route configuration update
started sending traffic to a new cluster.
xds: match virtual host domains case-insensitively (#9399)

The xDS virtual host matcher compared a RouteConfiguration domain
against the request :authority with case-sensitive equality, prefix, and
suffix checks. Host names are case-insensitive and the :authority is
chosen by the peer, so a mixed-case authority like FOO.BAR.COM never
matched a domain configured as foo.bar.com. On the server data plane the
request then fell through to a broader domain such as "*", which can
carry weaker route or RBAC policy than the specific virtual host it
should have selected.

The domain matching logic was also duplicated between the xDS client
(internal/xds/xdsclient/xdsresource/matcher.go) and the server
(internal/xds/server/routing.go). This change consolidates it into a
single exported xdsresource.FindBestMatchingVirtualHostIndex, which
returns the index of the best matching virtual host and folds the
authority to lower case once at entry. The xDS dependency manager uses
the index to pick the virtual host from the update. The server keeps the
update's virtual hosts on usableRouteConfiguration alongside the
interceptor-carrying copies and indexes into those with the result, so
the per-RPC path does no allocation. Virtual host domains are folded to
lower case once when the RouteConfiguration is unmarshaled, covering
both RDS and inline route configs from LDS, so the matcher itself no
longer touches case.

RELEASE NOTES:
* xds: match virtual host domains against the request authority
case-insensitively
server: release mutex while waiting for handlers in stop() (#9394)

Fixes #9393

`Stop()` deadlocks when called after `GracefulStop()` while a handler is
still running.

#### Root cause

Both `Stop()` and `GracefulStop()` call `stop()`, which acquires `s.mu`
and holds it for the entire function, including across
`s.handlersWG.Wait()`. If a handler ignores context cancellation,
`GracefulStop()` blocks in that wait while still holding `s.mu`. A
concurrent `Stop()` (the documented way to abort a `GracefulStop()` that
is taking too long) then blocks forever on `s.mu.Lock()` and can never
force the server to shut down.

#### Fix

Release `s.mu` around `s.handlersWG.Wait()` and reacquire it afterwards,
mirroring the connection-drain loop that already releases the lock via
`s.cv.Wait()`.

This is safe because none of the state guarded by `s.mu` is accessed
during the wait: by that point listeners are closed, all transports are
drained/closed, `s.conns` is `nil`, and the worker channel is closed.
`handlersWG.Wait()` only touches the `WaitGroup`, which has its own
synchronization and was never guarded by `s.mu`. A concurrent `Stop()`
can now acquire the lock and return promptly.

#### Testing

Added `TestStopAfterGracefulStopWithRunningHandler`, which starts a
handler that ignores context cancellation, closes the client connection
so the transport drains, runs `GracefulStop()` in a goroutine, and
asserts that a subsequent `Stop()` returns promptly. Verified the test
times out (deadlocks) without the fix and passes with it.

RELEASE NOTES:
* server: fix deadlock where `Stop()` blocks indefinitely when called
concurrently with an in-progress `GracefulStop()` waiting on a handler
that does not return promptly
server: Remove service from updates if no streams remain [leakfix] (#9411)

## The bug

`health/server.go` (v1.73.0, lines 107-111):

```go
defer func() {
    [s.mu](http://s.mu/).Lock()
    delete(s.updates[service], stream)
    [s.mu](http://s.mu/).Unlock()
}()
```
Currently we accept a service name and unconditionally add it as a key
to an in memory nested map (that is maintained for the lifetime of the
process). Upon receiving an unauthenticated watch request, we
immediately add that hitherto unvalidated service name into the map.
Upon disconnection, we remove the stream objects from the inner map, but
fail to remove the keys from the outer map, leaving behind those
unvalidated strings as keys in the outer map for the lifetime of the
process..

Potential impact:
Unauthenticated users can leave behind very large strings that the
process will maintain references to until the process restarts. The
strings can be up to 4 MiB in size per request, leading to very rapid
memory exhaustion of the service in the event of an attack from a
malicious actor.

## Fix

Delete the inner map when it empties, in the cleanup that already holds
the
lock:

```go
defer func() {
    [s.mu](http://s.mu/).Lock()
    delete(s.updates[service], stream)
    if len(s.updates[service]) == 0 {
        delete(s.updates, service)
    }
    [s.mu](http://s.mu/).Unlock()
}()
```

Verified against a patched v1.73.0: the same 200 cycles end at
`services=0`
with ~4 MiB of growth, down from 200 and 208.4 MiB.

RELEASE NOTES:
* `health/server.go`: Address resource leak, remove service entry in map
when empty.
xds: lowercase RDS route header matcher names before matching (#9353)

Fixes #9352

xDS RDS route header matchers store the header name verbatim from the
config in `routesProtoToSlice`:

```go
header.Name = h.GetName()
```

The matcher engine looks that name up against `metadata.MD`, whose keys
are always lowercase, so a matcher configured with a mixed-case name
such as `X-Role` matches no header and the route never fires.

This is the last consumer of that matcher engine that does not normalize
the name. The RBAC HTTP filter started lowercasing it at parse time in
#9332 (`normalizeHeaderMatcher`), and `authz/rbac_translator.go` does
the same on the non-xDS path. Envoy stores `HeaderMatcher.name` in a
`Http::LowerCaseString`, so this is valid config that works on other
proxies.

The fix lowercases the name where the route header matcher is built,
matching the sibling paths. Added a table test covering a mixed-case
name against lowercased metadata, and asserting the parsed matcher
stores the lowercased name; it fails on `master` and passes with the
change.

RELEASE NOTES:
* xds: fix RDS route header matchers with non-lowercase names never
matching
metadata: Add ValueFromOutgoingContext (#9282)

Partially Fixes: #8860

### Why

There are use cases where only a single value needs to be read from
outgoing gRPC metadata. Today the only way to do that is
`metadata.FromOutgoingContext`, which merges and copies every header
already staged into a brand-new map, even though the caller only wants
one of them. The cost of that copy grows with however many headers have
already accumulated in outgoing context — exactly the complaint raised
in #8860.

### What

This PR adds `metadata.ValueFromOutgoingContext(ctx, key) []string`,
symmetric to the existing `ValueFromIncomingContext`, for reading a
single header from outgoing gRPC metadata without merging and copying
every other header already staged via `AppendToOutgoingContext`.

This is the API @easwars suggested in
https://github.com/grpc/grpc-go/issues/8860#issuecomment-3975157332:

> Would adding a `ValueFromOutgoingContext` that is similar to
`ValueFromIncomingContext` work for you? Please note that
`ValueFromOutgoingContext` will not be as fast as
`ValueFromIncomingContext` as the implementation would have check
entries from the `map` **and** the `added` entries.

per https://github.com/grpc/grpc-go/issues/8860#issuecomment-5121951898.

That tradeoff is exactly what this implementation does: it checks
`rawMD.md` (case-insensitively, matching `ValueFromIncomingContext`'s
semantics) and then walks `rawMD.added`, accumulating matches in the
same order `FromOutgoingContext` would, without allocating a new map or
copying unrelated keys/values.

Matching `rawMD.added` entries case-insensitively via
`strings.EqualFold`, rather than assuming they're already lowercased by
`AppendToOutgoingContext`, makes no assumption about how `rawMD.added`
was populated — the same guarantee `FromOutgoingContext` already makes
for both `rawMD.md` and `rawMD.added`.

The allocation of `vals` is deferred until a match is actually found in
`rawMD.added` (per `gemini-code-assist`'s review suggestion), rather
than always calling `copyOf` as soon as `rawMD.md` matches. When the key
is found in both `rawMD.md` and `rawMD.added`, this avoids paying for a
`copyOf` allocation that would otherwise immediately be discarded by the
first `append`'s growth — roughly halving allocations for that case (see
`key-found-in-md-and-added` in the benchmark below).

The `rawMD.added` loop also tries a direct `==` before falling back to
`strings.EqualFold` (a second `gemini-code-assist` suggestion), since
`AppendToOutgoingContext` already lowercases keys in practice.
Benchmarked with realistic-length keys (e.g. `"grpc-timeout"`, not
`"k1"`), this is a modest win when the key is found (~2-7% faster) at
the cost of a small regression when it isn't (~6% slower — one extra
comparison with no payoff) — a reasonable trade since looking up a
header you expect to be present is the common case.

### Benchmark

`n` is the number of unrelated headers already staged (one
`NewOutgoingContext` call plus one `AppendToOutgoingContext` call)
before reading the target key:

```
goos: darwin
goarch: arm64
pkg: google.golang.org/grpc/metadata
cpu: Apple M4 Pro
FromOutgoingContext/n=1         141.7 ns/op    432 B/op    4 allocs/op
ValueFromOutgoingContext/n=1     56.0 ns/op     16 B/op    1 allocs/op
FromOutgoingContext/n=10        404.9 ns/op    968 B/op   15 allocs/op
ValueFromOutgoingContext/n=10   107.2 ns/op     16 B/op    1 allocs/op
FromOutgoingContext/n=50       1596.0 ns/op   3592 B/op   55 allocs/op
ValueFromOutgoingContext/n=50   314.7 ns/op     16 B/op    1 allocs/op
```

(measured with `b.Loop`, per review feedback, which also removes the
need for the manual anti-optimization `b.Fatal` checks the previous
numbers were measured with)

At n=50, roughly 5x faster with 55x fewer allocations. This is still
O(n) in the number of already-staged headers in the worst case, since
`rawMD.added` isn't indexed by key, but it avoids the wasted work of
copying and lowercasing every header the caller doesn't want.

Separately, `BenchmarkValueFromOutgoingContext` (now using
realistic-length keys like `"grpc-timeout"`/`"content-type"` instead of
`"k1"`/`"k3"`, which understated `strings.EqualFold`'s cost) shows both
`rawMD.added` optimizations above:

```
key-found                    59.6 ns/op    16 B/op   1 allocs/op
key-not-found                50.7 ns/op     0 B/op   0 allocs/op
key-found-in-md-and-added    38.7 ns/op    32 B/op   1 allocs/op
```

### Testing

- `TestValueFromOutgoingContext` covers exact match, case-insensitive
match, a value present in `rawMD.md` accumulated with two later
`AppendToOutgoingContext` calls (must match `FromOutgoingContext`'s
order, per `TestAppendToOutgoingContext`), values split solely across
multiple `AppendToOutgoingContext` calls, not-found, and
no-outgoing-metadata-at-all.
- `TestValueFromOutgoingContext_AddedCaseInsensitive` constructs
`rawMD.added` directly with a mixed-case key (bypassing
`AppendToOutgoingContext`'s lowercasing) to verify the match is still
found.
- `TestValueFromOutgoingContext_PanicsOnOddPairs` covers the defensive
panic on a malformed `rawMD.added` entry, mirroring the identical guard
already present in `FromOutgoingContext`.
- `BenchmarkValueFromOutgoingContext` mirrors the existing
`BenchmarkValueFromIncomingContext` shape (key-found / key-not-found),
plus a `key-found-in-md-and-added` case covering the deferred-allocation
path above.
- `BenchmarkValueFromOutgoingContextVsFromOutgoingContext` produced the
comparison table above.
- `ValueFromOutgoingContext` itself is at 100% statement coverage (`go
test -cover`).
- `go test ./metadata/...`, `go vet ./metadata/...`, and `gofmt` are all
clean. This branch is rebased on current `master`.

This is additive only — no existing exported behavior changes.

RELEASE NOTES:
* metadata: Add ValueFromOutgoingContext, which reads a single metadata
value from outgoing context without copying the entire outgoing metadata
into a new map.

---------

Co-authored-by: Claude <noreply@anthropic.com>
xds/rbac: lowercase header matcher names before matching (#9332)

The RBAC filter passes the name of a header matcher to the matching
engine unchanged. The metadata that the engine matches against always
has lowercase keys. A name that contains an uppercase character
therefore matches no header, and the rule that holds it never fires. The
policy parses, reports no error and looks active. A DENY policy written
this way fails open.

The A41 validation reads the same unnormalized name, so the rejection of
:scheme and grpc- prefixed matchers misses the name Grpc-Status. A Host
matcher also keeps its name, although A41 makes host and :authority
equivalent.

Lowercase the name in normalizeHeaderMatcher. That function already owns
the A41 rules and already rewrites the name in place, so the matching
engine, the :scheme and grpc- rejection, and the host alias all read one
normalized name. Envoy holds each header matcher name in a
LowerCaseString, grpc-java lowercases the name before it looks the
header up, and authz/rbac_translator.go lowercases the name on the
non-xDS path.

The new test in test/xds shows the effect on an end user. A DENY policy
on the header name User-Agent returns OK for every RPC before the change
and PermissionDenied after it. The unit tests cover the name that the
parse gives to the engine, the case of the :scheme and grpc- rejection,
and the host alias, at the top level and inside a nested rule.

RELEASE NOTES:

- xds/rbac: Fix a bug where a header matcher whose name was not
lowercase, such as `X-Role`, matched no header, which could cause DENY
rules to fail open.
- xds/rbac: Fix a bug where a `:scheme` or `grpc-` prefixed header
matcher was accepted when its name was not lowercase.
- xds/rbac: Fix a bug where a `Host` header matcher was not replaced
with `:authority`.

Co-authored-by: Markus Magnuson <331091+alimony@users.noreply.github.com>
client: don't reuse a pick's authority override on a later attempt (#9277)

`csAttempt.newStream` writes the gRFC A81 authority override from the
pick result into `cs.callHdr`, which is allocated once per
`clientStream` and shared by every `attempt`. Nothing clears `Authority`
between attempts, so once an attempt is picked to an endpoint that
carries a hostname, a retry whose own pick supplies no override still
goes out with the earlier endpoint's authority. An xDS cluster using
`auto_host_rewrite` whose endpoints do not all set hostname hits this on
any retry, and `:authority` is what selects the virtual host and what
RBAC host matchers run against on the receiving side.

Copy the call header per attempt so the override, like
`PreviousAttempts`, stays with the attempt that produced it. The value
is derived from that attempt's pick result, so the attempt is the right
scope for it; leaving it on the stream lets one endpoint's rewrite
outlive the pick that asked for it.

RELEASE NOTES:
* client: Fix a bug where a retried RPC could be sent with the
`:authority` value belonging to the endpoint chosen by an earlier
attempt, when a load balancer supplies an authority override (e.g. xDS
`auto_host_rewrite`)"
