ci: one Claude review per PR at a time, at effort high

A new push now cancels the stale in-flight review instead of stacking a
second run on the same PR. Effort max on every synchronize is the only
place the routine review needs the top setting; high is the documented
default. Drop the transcript dump that was marked temporary.
