pw_rpc: Move more functionality to the Endpoint class - Move the channels list from the client to the endpoint so they are guarded by the Endpoint's intrinsic lock. - Consolidate the logic for creating a call object and invoking an RPC in the Endpoint class. - Create custom exceptions to differentiate between channel, service, and method errors. - In MethodClient, handle the case of a channel or method being removed from the client by throwing an exception.
